WRAP-AROUND MENTAL HEALTH & WELLBEING SUPPORT FOR YOUNG PEOPLE AGED 16-25
What we do:
Invictus Wellbeing are commissioned by the South West Yorkshire Partnership NHS Foundation Trust to deliver the Open Minds Transition Support Service.

This service delivers wraparound mental health & wellbeing support to young people, providing a low intensity and less formal level of support.

How we work:

Each young person is assigned a Wellbeing Practitioner or Therapeutic Practitioner (dependent on their needs) who will support them on their journey from adolescence into early adulthood if they have struggled with mental health and/or everyday life. This support is built to improve mental health & wellbeing, resilience and daily coping skills.

This support reflects what young people have told us they want in wraparound support and will include:

- Person Centred Counselling
- Therapeutically Informed Support
- Welfare Check ins/Appointments
- Peer Support Groups
- Workshops, Sports and Creative Activities
- Volunteering Opportunities & Social Action
- Social Prescribing & Active Signposting

How do young people access the support?
